Manage courier forms

Courier forms are short forms your drivers complete on their route in the Nash driver app — a check-in before they start, a check-out when they finish, or a summary after a pickup. You build and manage those forms under Fleet ▸ Forms.
Note
Fleet and the Forms page are gated. Availability varies by organization — if you don't see Forms under Fleet, reach out to Nash. Forms is not available to scoped (limited-access) users.
Where forms live
Select Forms under Fleet in the sidebar to open the list of forms for your organization. Forms you build here decide what a driver sees and fills in during their route; the driver completes them in the Nash driver app, which is covered in the separate driver-app help.
The forms list
Each row is one form, with these columns:
- Name — the name you gave the form.
- Form type — when the form is shown to the driver: Check-in, Check-out, or Pickup Summary.
- Fields — how many fields the form contains.
- Filter criteria — which driver groups the form is limited to, shown as
Driver group (n). A dash (—) means the form applies to every driver group.
Select any row to open the form in the editor. See build and edit a form for the full editor.
Create a form
Select Create at the top of the list to open a new, empty form in the editor.
Delete a form
Open the row's actions menu (the ⋯ button at the end of the row) and select Delete. A confirmation dialog appears; select Delete again to confirm. Deleting a form stops drivers from seeing it on their next route. Submissions drivers already made are kept.
