Whether you see any of these forms, and exactly what fields they ask for, is entirely up to your operator — they build the forms, and the fields vary from one organization to the next. This article covers the general path for filling one out, not a specific operator's form.

Some operators ask you to fill out a short form at certain points in your day — before you start, after you've loaded up, or at the end of your route. These forms are built by your operator to capture whatever they need to know, so what's on them varies by organization. This article covers where you'll run into them and how to fill one out.

Check in before you start

Many routes start with a check-in — a short form your operator has set up that you fill out before you begin. If your operator has one configured for your route, you'll be prompted to complete it before you can continue; if they haven't set one up, you'll see a message letting you know there's nothing to fill out and you can move on. The fields on a check-in form are entirely up to your operator — it might ask about your vehicle, your readiness to start, or anything else they've chosen to track.

Confirm your pickup summary

After you've loaded up at a pickup, some operators ask you to complete a pickup summary before you head out. This can include things like the categories or weight of what you're carrying, a truck or vehicle registration number, and who you dealt with at the pickup location. Many pickup summaries end with a signature confirming that everything was loaded correctly, so read it over before you sign — you're confirming the load is accurate.

Wrap up at the end of your route

At the end of a route, you may be asked for quick feedback — a rating of how the run went, a rating of the app itself, and any tags that apply. This step is usually short and skippable. Depending on your operator, you might also see other end-of-route steps, like logging your final odometer reading.

How to fill one out

When a form comes up, work through it field by field — text entries, selections, checkboxes, or a signature, depending on what your operator has asked for. Submit it once everything's filled in; if something required is missing, you won't be able to move on until it's complete. If a form doesn't look like what you expected, or you're not sure what a field is asking for, check with your operator — they control what's on it.
